How do I know an answer is trustworthy and not made up?

Because an answer is built only from passages retrieved out of your own content, and each one comes back with the exact source you can open and read, you are never taking the AI's word for it. On top of that, the platform grades its own answers against those sources on live traffic, not a one-off benchmark, so if grounding starts to slip it is flagged before your people feel it.

Can the AI leak information a user is not allowed to see?

No. Before any passage is used it is checked against that specific user's rights, so a source they are not allowed to see is simply left out, as if it were not there, even when the answer is stitched together from many documents at once. The same check re-runs at each hop when the AI follows a trail from one document to the next, so nothing slips across a boundary partway through a multi-step question.

What happens to answers when a document changes or access is revoked?

Two things happen. A change to permissions in your source system takes hold on the next query, with no reindex, so a newly restricted document stops being surfaced right away. And because your knowledge is kept across several coordinated stores built to come back intact after a reset or failure, an outage or restart does not quietly lose the answers your people rely on.
